What is the Transitional Aged Youth (TAY) Coordinating Committee?
Transitional Aged Youth

This committee creates a local service delivery system of select providers for the purpose of delivering a seamless, client-centred mental health and/or addictions service for transitioning youth, ages 16 to 24 that is built on breaking down barriers and building current and new partnerships. The emphasis is on addressing client-centred needs and earlier identification of clients to transfer and earlier linkages to adult services. The Transitional Aged Youth (TAY) Coordinating Committee reports to a Steering Committee.

What is the Referral Process?
  • A Transitional Aged Youth (TAY) Referral Form is to be completed by the case 
    manager/counsellor/most responsible person with client’s input which will include physical, psycho-social, client strengths and risks. It is important to complete the “Identified Needs” section (at the bottom of page two).
  • Any questions as to the appropriateness of the referral are to be directed to the Transitional Aged Youth Coordinator.
  • The completed Transitional Aged Youth Referral Form will be sent to participating agencies one week in advance of the Transitional Aged Youth Coordinating Committee, (which is held on a monthly basis), in an effort to plan for his/her transition into adult services.
