Transitional Aged Youth

The TAY Service Coordination service is a Mississauga-Halton LHIN initiative that provides a monthly place where agency staff come together to problem solve and assist youth who may benefit from an enhanced service plan and to successfully bridge youth into adult services.
The goal of the TAY program is to provide an active response within 48 hours of first being contacted

Ready 4 Life

Our Ready4Life program is the pillar of the youth programs, offering community-based support and providing system navigation and advocacy to link youth to needed services and resources. Our case managers are mobile, meeting with youth in their preferred surroundings within the community, rather than requiring youth to attend in-office appointments. They assist at-risk youth in meeting the needs of all areas of their lives and help them get the medical, financial, emotional, employment and recreational supports they need. Youth Supportive Housing

Supportive and affordable housing is available in a range of housing models, from a shared, or congregate living with medium support with staff available at regular times each day; to low-support housing where youth live independently and manage their homes, practicing the skills of independent living without ongoing staff support.



Just Be You

Just Be You is a youth led group providing social rec and peer-support for ages 15-25, experiencing mental health and/or addictions issues. This program allows young people to get connected with others in a fun, safe, laid-back environment. Just Be You runs every Friday at Oak Park Neighbourhood Centre, Oavkille from 7 PM-10 PM.  Contact:  905-805-7323



Distress Telephone Support

provides a confidential 24 hour, 7 day a week Distress Support Line to better support the people who we serve, with emphasis on after hours assistance for:

When you are in emotional distress
Your SHH housing issues (i.e. power is out)

We listen, we empathize, we are non-judgemental and we are solution oriented. You do not need to be alone. We are here to help.
